Accident reporting forms

NOTE TO LAW ENFORCEMENT: In a collaborative effort between the Minnesota Office of Traffic Safety and the DNR Enforcement Division, crash systems have been integrated into MNCrash. Law enforcement officers no longer need to complete the paper DNR form when investigating a crash involving an off-road recreational vehicle or a snowmobile. Effective immediately, online MNCrash should be used.
